Talk about feeling like an moron....

I had film broke at last frames while I was skating in Ottawa on -28C evening.
It was in M4-2. I can't remember if I asked at RFF or googled it. But solution was to wiggle the take-in tulip. Eventually film started to slide down.
 
I feel your pain. This last winter (July down here) I shot one of my best rolls of film in my OM4 when there were very large seas along the coast. There was a group of teenagers sitting together on the rocks who didn't notice me capturing their excitement and interactions as the waves crashed in the background. When I got to frame #38 I thought "there doesn't seem to be much resistance to winding on"... the leader hadn't engaged with the take-up spool so I had a blank roll. Talk about feeling stupid...

About a month later I opened the camera back without rewinding and lost half a dozen frames. I've become much more careful since then! Hope that makes you feel better 🙂

My dad's friend loaded his Mamiya M645 1000S backwards (paper side towards lens) and jammed it hard enough he had to take his camera in for disassembly to get the roll out
 
The first time ever I tried to load a roll of 120 film. Kept unwinding wondering where the hell would I find the film leader. Ended up loosening the entire roll.
 
Way back in 1968 I had taken my neighbor up on him getting a camera for me before his return from Viet Nam.
He got home before my camera and he loaned me his bitchin Minolta rig that he'd purchased while overseas. Black SR-T 101. I took it to Riverside raceway for the Times Grand Prix that year. Shot a roll of 36 and fell in love with his camera.
Got a blank strip of black film back from the developer. I didn't load it correctly and it never advanced. Still didn't kill my appetite for the SR-T or photography in general though.
I came to hate my Mamiya 1000 DTL though when if finally arrived. Never got a good shot with it and it was a brick compared to that Minolta.
 
I've shot an entire roll of film with a half-collapsed lens. It was even more stupid than forgetting it. I was bored waiting for a bus so I fidgeted around on the lens, collapsing it and extending it again. When I got it ready to shoot it was extended completely, but I had forgotten to latch it by rotating it.


The result: An entire roll of out of focus pictures, including some that looked like they might have been keepers had I not been an idiot. Duh!!
 
I have an Olympus Pen F that, at the end of a roll, will rip or break the film if you don’t pay attention to the counter and keep shooting past 72-74 exposures. At least I have a laundry room where I can open the back and load the film directly into a tank.

Whilst in Portugal on a trip, another traveler separated the film from the take-up spool. After they recognized the issue, I requested coats from a couple of people. Place the afflicted camera in the first, heaviest coat. Insert your arms into the sleeves the wrong direction. Wrap the coat up in a ball, and layer more coats on top. You should have a lightight bag at this time and proceed with care. Success.
