scanning problems...help!

I have developed a problem lately with my Epson 2450, I know it's old but it typically produces great medium and large format scans.

Lately I've been getting blown out highlights and some other wierd effects to my negatives (see below). The effect on this pic is along his arm (left side) and face.

I havent changed my developing, metering, scanning etc. I use a Mac with Vuescan, scan is Tri-X but I get the same results with Acros.

What do you think, is my scanner dying a slow death or is there an adjustment I can make that I'm missing.

Have you checked for dust inside the scanner / on the CCD device?

I am getting the same symptoms on my LS-50 and when opening up to clean the mirror noticed dust and stuff on there. Havn't given it a go since, but your symtoms look similar.
 
What does the histogram tell you?
Maybe the negs are to dense. When you push the CTRL button vuescan shows you the density.
I don't know your scanner but maybe it has a very low Dmax.
