Is it easy to focus with it on Bessa R/R2/R3 with it's quite short RF base?
Is it good as portrait lens? Can you show some portraits shot with it?
What are alternatives (85-90mm?), maybe from Zeiss fame ?

there are times when you need to carry the hood. But, I like the flare, I think it worked nicely here.

I had one for a short while. Sharp, yes, but too big and too slow.

It's sharp and can be focused on the short baseline bessas. That's why it's only f3.5, to keep it within the bessa's capabilities.
I don'think of it as large...except as any long lens feels large on a RF camera.
That said, I never warmed up to telephoto for RF, so my cv 90mm is for sale right now. I sold the 75mm after trying that. It's not the lenses, it's just that they don't fit for me.

I have a 90 Apo-Lanthar on my Bessa R: it's probably my sharpest lens, and very nice to use. My only complain is that the 90mm borders in the viewfinder are very small (all that stuff all around is very distracting), so I bought, for a few euros, a russian 85mm viewfinder, that is useful to concentrate my mind on the subject

Superb lens. I use it very little for portraits, but then, I don't do many portraits. I have never had any trouble focusing it on a Voigtländer R-series (I've not tried the R4) or T, or with a Leica or ZI. From personal experience, a 90/2 or 75/2 on a Voigtländer is definitely pushing your luck at close distances and full aperture.
The size Bill complains about is to a considerable extent the result of the huge lens-shade; but as I always use mine with the hood on, I can't say how essential this is. Probably not very, in most situations.
