Any Insight As To How This Happened?

Looks like the film was touching itself during processing. This happened to me with a 120 test strip I had loaded wrong. Is the area opaque? Attached is a reflective scan of my contact sheet. If you were to scan the negative the whole mess of an area would be white due to no light passing through it.

Could you have bent the film? I know I ruined a few shots when the film got stuck when I was loading once (old paterson plastic reel that was TERRIBLE). I crinkled the film there. I forget if it ended up putting black lines on the film or white but it was a similar effect

I think my old Paterson reels are also terrible - it has gotten to the point that I can never get the film completely loaded on the reel.

I just bought a new Samigon tank with "foolproof" reels. I will try it out this weekend if I don't die of heatstroke first.
