I don't have a receipt for a cosmetically mint Rigid Summicron that I want to have the haze cleaned off.

When shipping by USPS or UPS, do they require a receipt if the item is lost or damaged?

If you mean a shipping receipt, not necessarily.

For me, UPS only needed the item number.

If you mean a receipt for the purchase of the item you sent, then I don't think so. You may have to provide proof of the item's value however, and a receipt would be the easiest way.

As an aside, not sure about USPS but with UPS it can be difficult to file a damage claim because you need to provide evidence the item was packaged properly and in accordance to guidelines. If a package looks squished when it arrives, take photos before/while opening it up for inspection.

The way I'm reading your question goes like this...
Do I need a receipt to show the Post Office the value of an item I'm shipping to insure it for that value...??? I believe the answer is No...
Pretty sure UPS is the same way...I've never had to show what's in the box or a receipt for it's value...

The price of the insurance fee is based on the declared value...

The only receipt you need is the one showing you shipped the item and it was insured...if an item is lost or damaged...

And with UPS, at least here in Canada, if the item is damaged and you packed it tough luck. It's either they pack it or they lose it entirely for insurance to be worth anything.

Shippers know their stuff as they deal with frequent fraudulent claims.

For anything irreplaceable or of great dollar value it is wise to
have a sales receipt or appraisal as well as photos just in case.

Regardless of how much you insure your item for shipper will only
pay claim to amount item is actually worth i.e. replacement cost.

In the event of damage failure to comply with shippers packaging
requirements will result in claim not being paid.
Cushioned packing must be equivalent to the items volume on all sides.
i.e. a 4" cube should be shipped in a minimum 12 X 12 X 12" carton,
surrounded completely by appropriate packing material.

I spoke with Youxin Ye as I was shipping a M2 with a rigid Summicron which are both mint for CLA and cleaning of haze. He agreed with you saying that if there was damage, an independent appraisal was necessary to claim full value with the USPS. I find this rather tedious but given the level of dishonesty rampant in our society, it at least keeps insurance rates down.
