We can simplify it.
Anyone know a metal tube stockist in the UK?
We need thin walled tube with an ID of 36mm and also tube with an OD of 36mm.
The ID 36mm tube fits over the Elmar. The OD 36mm tube slides inside and the cut out plate is attached to it.
The plate with the cutout is easy. I will ponder.
Mind you I don't know how to make the aperture markings!
Pah, never mind all that, send me $350 and you are welcome to my hood, gift wrapped with a pretty bow. At that price I might even throw in a camera and lens!

I wonder what SK Grimes would charge to make something like this. I suspect it would be less than $350....but maybe not.

If not of course why not a 3D cylinder that fits on to the Leitz filter.

Years ago a well known magazine was so annoyed by the price of a replacement plastic part for a car that it asked a silversmith to quote for making one in solid silver. Needless-to-say, the silver one was a lot cheaper.
One of my family and friends circle is a well known jewellery designer; she reckons she'd make more money doing plumbing which is very similar to silversmithing...
