LTM-to-M adapter question

I have a Bessa R4 M-mount camera and one M-mount lens. I'm thinking about buying one of the CV LTM lenses w/LTM-to-M adapter. Does the adapter stay fixed to the lens, or to the body? I'm hoping the adapter stays on the lens so that when I want to switch to the M-mount lens in a hurray, I don't have to also fuss around taking the adapter off the body.

And one more "newbie" question: Doesn't the adapter move the lens just a smidgeon farther away from the film plane? Why doesn't that make focusing inaccurate?
 
If you screw it on hand-tight, it will stay on the lens, no worries, Jamie.

To answer your 2nd question, you use it to adapt lenses that are made for the
Leica Thread Mount standard. The LTM standard has
just about 1mm more registration distance than the M mount standard
(used for your R4). Exactly how thick the adapters are, so no focusing problems.

Nah, it has exactly 1mm more relief. Being centered at 12 o'clock on the adapter is aesthetically preferred but has no impact on focusing
 
For 17 years I used only screw lenses with an M3. Not once did an adapter come off a lens. You need to be careful not to damage your fingers, though.
