my winding lever came loose?

Can someone assist me on how to put the lever back? few days ago, the black cover cap to the winding lever just popped off on my R-D1s for no reason.

after the cap came off, the the metal washer came off, thus the lever now is loose and i cant seem to figure out how to put the wash and cap back on?

is there a PDF manual of some sort of how to put this back?
