I was googling today and found a company that makes brand new eveready camera cases for vintage cameras -- apparently for people who have more money than sense: http://www.kameraleder.com/index.html

Ah yes, the Never Ready case! Aren't there enough hardly used GENUINE Leica cases at church rummage sales to satiate the market? You need to have at least one in black and one in brown to go with your outfits and maybe treat your Leica to a new skin from CameraLeather in case it wants you to take it to the nude beach.
My cameras are always nekid! The gurls dig lookin' at the battle scars. It turns 'em on!

As a Kiev dyno, I do appreciate the intrinsic value of a leather case. I am not saying that the old kiev leather cases are the best, but this I can tell you:
a) The best EVER READY camera leather should be composed of two different leathers. A hard leather for the lower case, in order to silence the shutter. And a soft yet thick upper case leather, that once you are on the field you just take out and shrink into your pocket. I have done such combination with the Pentacon six.
b) Like in the early Kiev or Contax lower cases, the lower case MUST cover the FULL eyepiece, so that your eyeglasses never contact but leather.
c) Any decent strap must include swivels, so that when the strap wents twisted it will return to its right position just by you lifting the strap and camera.
d) A further note about the strap. It will make a big improvement if it is composed of two parts breaking the single continuity: a short part starting from the camera ear upwards, ending in a connector ring or whatever, and then the main strap connecting to the small piece through a swivel device.
This break into two parts is of high help once you twist your camera for a portrait shot, making the undesired excess of strap highly manageable, due to the breaking point.
e) Although this new company has done none of these, they have done something good for the Nikon and it is taking into account the space for a reversed hood.
f) Last word about the strap. Instead of progressively thickening it, some material avoiding the camera to slip from your shoulder should be there.
Cheers,
Ruben
