This strange shoe adapter came to me along with a viewfinder. It is very nicely made and even has star-bit screws. So I don't think it's a cheap Chinese thing.

Looks to me like a well made modification to attach a cold shoe to a camera capable of having a large screw or bolt on the front, I'd be guessing a 4x5 field camera or similar.
The item is well made of adapted parts and made within the last 10 or so years based on the screws used. The style also suggests that the maker would be prepared to neatly modify their camera to suit this part.
I'd say it came from wherever you bought it ;-)

That's a good thought, and very possibly true. I was assuming commercial manufacture. In to the parts box it goes.
