Just wanted to share some recent experience. I know that both Malcolm Taylor and John Milich have been mentioned before, but here's some recent feedback.
John Milich provided me with two LTM-M adapters both coded (for a CV 28/1.9 and 15/4.5). Superb job, exceptional quality. Efficient service, no flim flam, just quality.
Malcolm Taylor - especially convenient to those of us in EU/UK - coded my 28 Summicron and an LTM-M adapter for my Nokton 1.5. Again, superb job, exceptional quality. Great service, a pleasure.
I recommend both of them very very highly. If you are UK based, the current exchange rate makes Malcolm's work particularly good value, even though both are very reasonably priced offerings.
