All kinds of answers, half right, half wrong. under agitation from poor technique or poor immersion is the culprit 99% of the time.
Definitely try a remix in fresh fix. Can be done in a tray in room light, six frame strips.
Buy and tank and some bottles and thermometer and go for it. When you get better, you will want a one time dilution/use developer or even better raw chemicals and a scale. Then you have total control.
