Did you pull it fully out, then twist it until it stops? If it is correctly in place you can not then just push it back down into the camera; you have to twist it back the other way first.

Your lens may have been wacked out of its correct specs, infinity-collimating wise. Such things happen, especially with such old lenses which are very prone to have been disassembled and not properly reassembled.

How far out of focus was it? Do you have any in-focus parts in your images, or are the images just mushy goo?

Few were mushy and few had part in focus, one image was in focus as I recall!
The film was shot about 40 days ago, I'll try another film soon and will report back.

It worked when I locked it extended as you guys mentioned. It's working like it should.
I must thought that I locked it while it wasn't!!
