Actually, recently. I went for a walk a couple nights ago and was doing 1/4s exposures in available light with my M6. Some dude kept taking shots with his DSLR and pop-up flash on rapid fire. There is going to be some serious overexposure on the roll when I develop it!
 
Caught this one from the official photographer of my daughter's and son in law's wedding. They liked the photo better than many of her's... :)

Happened a few times as accident too, especially when there was a press pack around.
 
It's happened frequently to me when shooting weddings, specifically the bride and father dancing. Everyone seems to be taking photos at that point.

Looking for an example right now..
