I've just discovered that Epson Scan will overwrite files without any warning. This discovery has led to me changing my file naming system for scans.
The discovery happened when I realised I'd put an incorrect camera name as part of the file name string, and then tried to rename it in Finder (= Windows Explorer for PCs) eg. FE2_HP5_D76_R48_001.tiff renamed to F80_HP5_D76_R48_001.tiff. Finder gave me an error warning that the file name already existed.
I halted the batch scan and renamed the output file to avoid future conflict. Then I discovered that the original file had been overwritten by the new scan. Of course I still have the original neg and can re-scan it.
This is annoying - I would've expected Epson Scan to give warning if an output file name conflicts with an existing file in the same target directory.
Also annoying is that I can now only run Epson Scan on an older Mac running Snow Leopard. It doesn't work on Mavericks, as far as I can tell.
My file naming protocol now starts the file name with YYMM. That should avoid future problems.

EDIT: there is a check box in Epson Scan that will avoid overwriting files - thanks for your help guys!
