Used pre IIIC ER cases always show up on evilbay for less than $35, buy one that`s a little worn out and cut the top off, I have two old cases like that with my IIIC`s in them when they are being used and clean like new ER cases when they are in storage

It's also easy to drill out the old connector and replace it with a press-stud; I did this on my M3 case (I practised first with a Kiev 4) - I guess the LTM is the same. THis is very handy for if you're chucking your camera in a suitcase or boot/trunk.
Cases for Fed I's and Zorki's work perfectly well.
Cases for Screw Mount Leica's are cheap on Ebay. I picked up a case for the IIIG for $1. I was the only bidder. The former owner had her name scribed into the finish. Nothing a magic marker could not take out.

One more thought: you don't have to cut the leather to remove the case-front: take a seam-ripper or a small pen-knife and carefully pop the stitching where the front/top is sewn to the bottom... that way, it can be put back together someday.
I have a couple where the leather was indeed cut...
