It seems like there is too much pressure here

Any help or advice would be much appreciated.

I got this camera from KEH few weeks ago. And it has happened consistently on past three rolls of film.

There had to have been a minor imperfection on the roll edge.

Just make sure the film is exiting the cassette on a tangent and it rewinds the same way. If the "pull out" is not tangent, you can get scratches in addition to tears.

This should be a very rare experience if the cassette is not rotating around when the back is closed.

Could you show us what it looks like with no film. This is to see what the guides look like. Also, is the rewind button releasing the sprocket?

I'd return it. I had a similar problem with my OM-2n.

I will try few more different rolls from a different batch. If that does not solve this problem, I will return it 🙁

Try cycling a roll of film with the back open. Run it all the way to the end and then rewind. Watch the sprocket advance carefully. Also, have you checked the pressure plate?

I did exactly that when I took the photo. Started rewinding with the back open just to see what's happening. It rips right were the film enters the cassette. The sprockets did not damage or rip film wholes.

What do I need to check on the pressure plate?

Well, if it rips film that has been advanced and rewound with the back open, it's not the pressure plate.

In your picture, right above where the spot from your flash is, I can see small deformations at the bottom of the sprocket holes. Find out what is causing those deformations and you know what is wrong.
