Looks like it wasn’t it the spiral properly and the developer couldn’t get to the film

Hmm, could be. Is it possible the plastic Paterson reel I'm using which I bought used has soaked in too much chemicals?

Was it at the end of the roll?

It's wasn't at the end. Now that I think about it the edge with the problem would not be touching the reel :\

Not correctly on spiral.
Had a neg just like that last night on a 120 roll. The end had curled in and touched the affected frame, hence preventing proper developement.
Got lazy and had not cut off the very end of the roll that always wants to curl.
Another humbling day...

Had it come out of the track it would be touching the film-side of the film in the next spiral, if that makes any sense, I used to do it all the time with 220

Can you see it on the negative, or is it just on the scan? What about the frame next to it?

After more scanning I noticed this... it looks like the same marks on other negatives. These are frames #2 and #25 from the same roll.
These are the first rolls I've developed shot with this M3... perhaps a problem with the shutter?

Update
After inspecting the negatives by eye I realized that I couldn't see anything that would account for the marks. The previous scans I posted here were done with a Nikon Coolsan V. I also have an Epson V700 and decided to try scanning them again and what do you know...
The marks are not there and I think the dark area at the top of the right side images is from the lousy film holder not holding the film flat. That image is at the end of a strip so I think it's curving in slightly.
