Just messing with two pics, forget the content and framing please. Woud just like your honest opinion on which image you think is nicer.
Both taken with 50mm lenses.

The first. Why? The intensity of the boy's eyes are distracting me on the 2nd print and drawing me in for the wrong reason -- perhaps I'm finding the expression unsettling and hard to look at? But definitely the 1st for me, gut instinct.
 
The second pic is somewhat disconcerting to look at, due to his intense/concerned expression and the lighting which is almost from below. It could almost be a frame from a horror movie. It would depend what the purpose of the picture is. If it is to recall the fear and emotions of a hospital stay, the second one would be more successful in doing that.
 
#1. The second shot is classic "horror" lighting, which combined with the distortion of features makes the boy look "HORROR"ble. As boring as some may think #1 is, it shows him in the best possible appearace, given just those two frames to look at. Only suggestion would be to dodge the tome of the left teat so it isn's so prominent.
 
the composition of the first one is stronger, the line of the pillow intersected by the right hand and arm and the curve of the left shoulder and dark mass of hair keep ones attention on the face quite well, i prefer the expression in the second; but then that's probably why you posted them
 
Agree with ferider's conditional comment. But, if the question is which is nicer in my opinion, I vote #1 for many of the reasons already given, but also because number 1 seems more representational to me, has better separation of cheek from pillow, and seems sharper (is number 2 a bit shaky, has less dof, or simply lens signature less sharp?)

I agree. The second looks just a bit scary. The crop seems to make the subject loom in the frame.
 
There is back lighting, butterfly lighting, split lighting, short lighting, loop lighting, and then there's Frankenstein lighting.
 
The second may be more arresting initially but for me the first has more potential with some work!

The second also appears to have missed the focus on the eyes a little.

no. no big question for everybody, and definately no big debate for others apart from myself. One is shot with a rigid cron, the other with my D700 and a 50 1.8 Nikkor. I know now which I like best but was just curious. Both where turned into b&w with alien skin in cs4 using neopan profile which I really like, j
