I wrap the leica strap around my wrist and hold my leica with my fingers on the right side of the camera when i am actively shooting.
However times when i am travelling to dangerous places , i use an everyready case and it works perfectly , i can open it up quick take shots close it back up and walk without worry

Around the neck - all the times. Usually a 35 or 21 lens - or if it is a one lens/one body day - the 40 mm f1.4 Nokton. The importance is not how you carry it - but that you carry it with you all the time!

I bring a bag to work every day. My olympus XA is always in the front pocket. Sometimes I put an M6 in a Domke Wrap in the bag as well, depending on my mood.
