I number my photos [camera code][roll number (omitted for digital)]/[sequential number][edit number].
This gives, for example, FD14/023E1, where FD refers to my FE2 (I'm sorely tempted to rejig that part of the system - I allocate them sequentially, so FE was given next to my Bronica), and the 14 refers to the 14th roll through that camera. The 023 represents the 23rd frame, and E1 the first separetly saved edit.
Other information about the roll is kept in a little Access database, so I don't need to put film types and dates in all the names.

Numerical year-month-day plus description for scanned negatives. It makes file sorting by date trivial, and the description provides keywords for an easy computer search.
E.g.
"2009-04-02 poplar tree shadows"
The negatives themselves go into a transparent page, also identified by year-month-day of processing, plus longer description of camera, film and processing.
