Image upload

I tried to upload an image today, but I have the problem that I make one upload and the image seems to be stored twice. If I delete one of them, the thumbnail of the remaining image is gone.

Looks like a database problem to me. Or have I done something wrong ?

I sometimes have the same problem, I just delete both photos and upload again. I think it's because of user error, at least on my part.
