iPhone app for exposure tracking?

My resolution for 2011 is to shoot more film than digital and really improve my film skills. :bang:
I'm bound and determined to really get a handle on exposing properly when shooting my M4P. To that end I'm wondering if anyone has found an iPhone app that would allow one to set up a template to record exposure info for each frame of each film. I've tried carrying around a small notebook but invariably I walk out without it or forget to dig it out of my bag. In contrast, the phone is always within easy reach and would be the perfect place to store the info.
 
Here is my suggestion, which requires minimal effort while shooting, but more effort later.

Use the voice recorder application instead. Stick to a script: "Camera OM-1, Film FP4+, Lens 50/1.8, Frame Number 3, Shutter 1/60, Aperture 8, Lighting Overcast, Subject My Cat."

I've done that, and it worked out very well, as long as I stuck to the script.

The thought of using the keyboard to enter this data in the iPhone while also attempting to shoot pictures was not appealing at all.
