First of all, thanks once again to everyone who offered suggestions. I finally resolved the issue.
It turns out that the Paterson tank that I had wasn't "locking" properly. When you turn to close it clockwise, it's supposed to click-lock into place. For some reason, my tank wasn't doing that (at least consistently). And because this was the first film development tank I've ever used, my dumb-ass self didn't know differently. A new tank has resolved the problem (so tetrisattacks guessed correctly).
By incredibly good luck, none of my truly good keeper shots was lost to this problem... only the crappy filler stuff!
