Been carrying this for a while now.
Made by Will Leather Goods in Eugene, OR. It's not specifically a camera bag, but works great for an M or two (or three) with a couple lenses, a bunch of film, meter and misc. stuff.... Looks cool and built like a tank out of repurposed material.

Is it heavy? (looks like it might be). What model bag is it?

It is a bit heavy. It's solid leather with canvas covering the exterior. No plastic or nylon to be found. I'm used to carrying a big leather messenger bag full of art supplies so it doesn't feel that heavy to me.

They call it an Ammunition Bag. I just looked on their site and didn't see it offered anymore... Maybe I just missed it.

It's definitely not for everyone. It looked a hundred years old when it was brand new, but has aged well. I can see myself carrying it for a long time to come.
