Has anyone had problems with the M39 (ltm)-NEX adapter? My 50mm f/2 Summitar and 25mm f/4 Skopar screw in fine, but my 90mm f/4 Elmar and 35mm f/3.2 Canon don't full screw in (-1/16"").

I have 7 of these because I leave them attached in permanence to the lenses for quick interchange. Never had a problem with any.
My only observation is that because of tolerances in the aluminum casting I have to center the 39 mm ring. This is particularly useful with the wide CV lenses, 15 and 21mm, and the not so wide 35mm.
I use 0.003" thick stainless shims while tightening the screws. This is to center the vignetting although Cornerfix can take care of the unbalance. At the same time I rotate the ring to correctly position the lens scale.
I also generate the Cornerfix profile by shooting through a 6 mm thick milky white piece of translucent plexiglass uniformly illuminated by sunlight. I have generated much better profiles that way rather than shooting a gray cardboard or a white sheet or the sky.

After about an hour I was able to free up the threads, but the last 1mm of travel is somewhat stiff. At least the lens won't loosen off the adapter like some members have complained. I notified Fotodiox but haven't heard back yet.

I got three cheap adapters from Amazon vendors when I bought my NEX-5 a couple of years ago (the adapters were for Nikon, Canon FD and Leica screwmount lenses). All three worked on the NEX-5, and so I thought, why waste your money on more expensive adapters?
Then recently I purchased a NEX-VG30 camcorder and - wait a minute! - the one for Leica 39 works fine, but the other two wouldn't mount on the camcorder. I think I may look for more finely machined rings now.

For your information, the two adapters that won't mount both have the "Fotasy" name on them. The Leica screwmount one bears no manufacturer's name.
