Hi,
I've set the RD1s in order to take RAW+JPG, where JPG is B&W, hence having theoretically two shots, RAW in color and JGP in B&W.
Recently (it started all of a sudden, apparently) it happens that in a very random way, the camera takes two JPGs instead of the combo above, both in B&W!
Files are automatically named i.e:
27082017-EPSN1706.jpg
27082017-EPSN1706-2.jpg
Is it ever happened to you?

This is a good one, I've done the same RAW+jpg for years with my cameras and never had this issue. Try a reset or get the R-D1s upgrade.

Another nope...never have had this problem with the R-D1s. Have you tried to just go for RAW and Jpeg without setting color/b&w options?

This is what I am doing at the moment, it seems in fact to be somewhere in this options the problem... I'll keep you posted on this one.

I use the same RAW + jpeg b&w settings, no problems.
Reset all as Steve suggests, and try a different, fully charged battery. My RD is very battery sensitive.
