Rapid winder slippage

Has anyone hee noticed any problems with the film not advancing all the way whan using the R3a and rapid winder? I'll have 1-2 frames per roll that overlap when I use the winder. The shutter cocks fine. And it feels fine when I use the advance. Doesn't happen with the lever advance. I've had it off to check alignment but seems fine. Help please!

I get slippage with both the winder and the regular crank. This usually happens when I don't go through the full motion when winding. I've had it happen on a R2 and R3a including my last roll.

If you try to shoot and wind a sequence of shots as fast as you can, there's a likelihood of getting the left and right hands out of synch. Then you may hit the shutter button before the wind stroke is complete, and wind while the shutter is operating, and both can cause problems. When I tried that I did get a couple of frames overlapped. I later heard here that getting uncoordinated winding and shooting can result in jamming the shutter, so I was lucky that didn't happen.
 
Any updates on this?

I'm loving the winder and have it tightened and make sure and hit the stop when pulling the lever but I'm still getting one overlap every 2-3 rolls (36).

I havn't minded before but it ruined a nice shot from a wedding I went to at the weekend...

PS The overlaps are always two frames overlapping from their centres - never significantly more or less.
