scjrew mount adapter no longer twists into place

i have an LTM to M adapter made by Voigtlander. After years of faithful service, and no abuse that i can recall, it no longer slides over to click into place. It does not look warped, and i cannot for the life of me figure out what happened. Any suggestions?

I probably would start by being certain the adapter is clean and dry. Then try mounting it with out a lens.
As well, If you've got any M-mount lens, try that (or those) and see if it is locking into place as expected.
