How about the Petri Color 35? Yea, I know it's scale focus, but it's tiny.

I was going to say the Contax T (the original folding one with the real optical rangefinder) but I guess it doesn't have metered manual exposure.

Speaking of folders, how about a Zeiss-Ikon Contessa or a Kodak Retina IIIC? These have rangefinders and are metered manual, all right -- the meters are selenium, you read out the exposure on a dial, and then you manually transfer it to the camera controls! (Or you could call this 'digital exposure control,' since you have to grab the shutter and aperture rings with your digits...)
