For me: notebook and pencil;

In snow, ice or heavy rain: freezer bag for camera (and use indoors for warm-up time to prevent condensation).

Perhaps: tape measure.

For some of you; (small) trolley and assistant...

Pen, notebook, plastic bag and digital tape recorder (or whatever those are called). Some change in case you photograph street musicians and they ask you for money. Photo ID just in case.

Duct tape. Duct tape is one of the humanity's greatest inventions. It will hold your light stands in place, your lights, flashes, heck, I've seen it hold cameras together, too.
