What did I do wrong?

Attached is a scan from a roll I processed earlier today. The film is Kodak Tri-X shot at ISO 400 souped in D76 1+1 @ 22C 9 minutes. All the frames came out with that "stripe pattern". I've developed over 30 rolls and figured out how I messed up a couple but this eludes me. Any tips will be appreciated.

Is it a little bit milky? Then it's time to change your fixer. Just put the neg thru fresh fixer, and wash again. No harm done.

What kind of developing tank are you using?
This happened to me two or three times many years ago when I was using a primitive tank without a spiral , just kind of special tape to separate the film layers. This time I found it to be the uneven agitation the reason why it happened. Once I´ve turned to spiral fitted tanks or better SS spirals, never more!

Yes it was milky....I just mixed some fresh fixer and dipped/washed the negatives again. All is well now. Thank you very much!
