I'm asking a lot of really basic questions here, so I thank you for your tolerance.

When do you use a tripod?
Always
Only with medium format cameras.
Only with large format cameras.
Only with 35mm cameras.
Never
Only occasionally.

For me it's nothing to do with format but just when the situation requires it.
 
When do I use or rather when do I bring a tripod with me. A when I'll be shooting 4x5. B when I expect to be shooting in a location that has limited light.
 
Sometimes, with most formats (I even have a tripod adapter for my Minolta 16-II). Always, with the cameras I can't hand hold. I almost always use one in the studio, but less on location.

I'll use it often with my Mamiya RB67, I find that the weight of the camera and the slap of the mirror can lead to some issues. It's just heavy and I like to keep my horizons pretty straight which I tend to struggle with when holding the camera at waist level.

Of course with a field camera as well... a little heavy!
 
Most of my photography is hand held without a tripod but I do use a tripod at night for fireworks and time exposures using my 35mm digital equipment. I sometimes use the tripod with Polaroid, either for self portraits or when the light is low for the FP100C film. I did not vote because I don't see anything that fits 'sometimes for multiple formats'.
 
The only format related usage of the tripod would be LF, I guess.

The reason to use it is related to:
Preventing camera-lens from shake. Short and long terms.
Moving camera only in one dimension. Panning, for example.
You want to be in the picture you are taking.
Storage. Keeping it nearby, air show, as example.
Convenience. Operating it, like with very old folders.
