Use a billingham hadly pro, cost about $250 or so on amazon when I purchased it 3 years ago, if billingham's track record stays as good as it has been this should be my bag for a number of decades to come.
I like leather bags like Wotancraft's, which tend to be more expensive (it doesn't rain much here in Northern California). Nowhere near as much as the Leica Schedoni bag, available for a mere $4500...

My current favorite is a Filson camera field bag. It took me a while to find one heavily discounted on eBay but glad I waited.
thanks!!!
i love doctor bags...but i have never bought one, think i'm afraid of starting another obsession that i cannot afford.
plus, it's hard to find smaller bags...yours look great!
FINALLY! Settled on two Filsons. The "Camera Field Bag" with Domke inserts and the "Harvey Backpack" which I use for other things not photograph, but was my backup supply bag this past weekend. The field bag is about as perfect a bag I could ever need for me. I carried a Fujifilm GA645, Lumix GH2 Kit, and extra supplies for both, and the Polaroid 420 Landcamera around my neck.
Bought both new on ebay for way less than retail.

I've been through a series of Crumpler bags - a backpack and a slew of messenger bags - all around the $120 mark, and they've been... ok.
My bag shouldn't cost more than any camera or lens I'll put in it.
You use the field bag with an additional insert? Just curious as to why, simply for more padding? I used the field bag briefly and returned it as I felt it collapsed on itself too much which might be resolved by adding another insert.
Curious as to how cheap you were able to get it on eBay.
