I think all my ltm-> M adapters are no name adapters, never a problem for me, save some issues every now and then with there not being cutouts for infinity locks. Other than that, Heavystar is pretty solid with everything they make
 
I use something similar from them on my GXR for L39 glass. No problems, but then, no rangefinder coupling either. I think I also got my FD/LM adapter from them as well.
 
Count me in on the never had a problem side.

The only annoying thing, which I have noticed on a few (I've owned tons of these as they are cheap enough I often include one with a lens if I'm selling a LTM lens) is that the threads aren't aligned to stop all that well. Now this may be a common thing across all adapters (Leica, Voigtlander etc) where the lens doesn't stop in the 12 o'clock position. It is usually around 11 with some of the cheaper adapters.
