Double exposure with Iskra

My Iskra allowed me to double expose one frame on my last roll of film thru it; this hasn't happened before. How did I do this? My shooting procedure is 1) wind 2) cock ) 3 shoot.
I suppose it wouldn't be out of the question for a 44 year-old camera to "burp" once in a while--- any thoughts?
 
While I owned mine, I had that happen on several occasions. I was never able to find anything that showed me how or why. I just kept shooting it until I sold it with the understanding that it could present me, at any time, with something "wonderful" :D

I'll have to give it a try, maybe the film didn't advance or skipped on the take up spool.

The funny thing is, I'm sure I wound on the film wind after each shot. The double occurred at the third exposure. I know I would have noticed if something felt amiss, but all seemed normal. Now--- as far as I understand, it is not possible with a (normally functioning) Iskra to forget to wind, then cock the shutter, and double expose that way, like with most "red window" MFs. Am I wrong? If I am correct in my assumption, then clearly I might have a problem with mine.
