Hi Doug - I have the full Adobe Acrobat product so I think the easiest thing to do is simply download a copy of the form and scan it directly into Acrobat and create the text fields myself.Doug said:Hi, Peter -- I just used the .pdf form as a background graphic and overlay the text... If I manage to find the time to do this I'll upload it to the forum so others will be able to use it. It would be a regular PDF form that you can enter data into and save on your hard disk.
