Help - dark spots on negatives

Need some help diagnosing the cause of an issue with my latest roll of film.

Camera: Mamiya 645 J (used often, no known light leaks or issues)
Film: Ilford FP4 120 (fresh roll from B&H)
Developer: Diafine (mixed a year ago; I usually use each new batch for 2-3 years; using Diafine since 2008 and never had an issue like this)
Fixer: Ilford (mixed 2-3 months ago, used on maybe 6-8 rolls)
Development: 3+3 per Diafine instructions, gentle invertions for first 15 sec., then one gentle inversion every 30 sec. There is no possibility that a foreign substance was splashed on the negatives during film development.

The photos below are straight scans from an Epson v500 with no editing. The white spots on the scans are visible as black spots on the negatives. The spots show up on all frames but are inconsistent in pattern and frequency. Some frames it is barely noticeable. Other frames it is overwhelming.

Unfortunately, its not just the fence. It is faint, but the spots continue into the leaves. Definitely, the spots are more visible in the midtones.
 
Ouch. You might send a negative to Ilford, they have been very helpful in the past diagnosing problems, but that was when Simon Galley was still there. I’ve not been in the situation since he left.
