Is it worth doing? I don't want to ruin the covering of the body of the camera.

tell us what camera. Some leathers come off easier than others. If its a Canonet you will not get it off to reuse it.

It wiggles a little side to side, and in a twisting motion. It looks like its the entire lens that moves. I believe theres a couple of screws under the leather, but is it worth ripping up?

I've got a Canonet service PDF if you want have a gander.
If the whole lens unit is loose (hold the camera body with one hand and gently shake the camera up and down, if you feel the lens moving, it's loose) it means the camera might have been dropped at some point. Which means the rings that hold lens are warped or damaged.
That covering on Canonets is very permanent, even more so after 20-40 years. You could always replace the covering with something new from Camera Leather. Like paradise pink

I may leave it as is, it hasn't had any affect on pictures, as far as I can tell. It wouldn't be worth screwing it up any more...
