If you google "model release for" a whole bunch come up. I use one that I took from the New York Institute of Photography. But I can't seem to find the link right now. Try this one instead: http://www.apogeephoto.com/mag4-6/adultlon.htm

I'm looking for something to use for for a project where I'll be soliciting photos from strangers on the street.. since they'll have to participate in a certain way, and their faces will be the focus of the image, I'll need their permission to use the photos in a book I plan to print through LuLu.. but I don't want the model/photo release to be too overwhelming or they might balk at participating
I want to cover my bases in the easiest and least intimidating way possible.. the link that Rafael provided should work well.. altho I might decide to merge that and yours into a concise, but encompassing document

You can change the $___ part to "photographs" and mail a photo to them. The book refers to the short form as a "pocket model release."

Model releases aren't required in order to include people in a book . . . even if the primary purpose of the book is to sell 50 million copies. Model releases are only required if the image is going to be used to advertise (promote) a product.casino guide

That's my understanding too, at least in the UK and US. You may prefer to seek consent, but you don't have to.
