Bought a new V500 a month ago, have scanned about 60-70 rolls so far. I am using it with the Epson Scan software in OSX. Lately I've been experiencing the following symptoms:
-sometimes, in the middle of scanning, I'll get an error that says the document mat is open, I need to close the lid. However, the lid will be closed. I need to physically shut off and restart the scanner.
-I'll hear a weird, more strained pitch when scanning - the scanning will finish but I'll get one frame that's completely white.
-Sometimes there will be weird digital artifacts in the final image.
Has anyone else experienced these issues? Should I try to get it replaced? I'm leaving the country for about a year at the end of next week, so I'd like to get this stuff fixed while the scanner is still in its warranty period.

Actually, I realized my mistake here...however I still keep on getting some of the errors above.

This is the result of scenario number 2, encountered just now. This one came in the middle of a batch of 12. The scanner made an abnormal, strained sound when scanning, and the result was very washed out. Later rescanned and totally normal.

Not had any problems so far with my V500.
Why not just return it though , if it`s within warrenty, and let them figure it out.
It would seem that you have taken reasonable steps to try and eleminate any user error.

That is true, would not really like to go through the hassle of shipping it back or taking it to a service center... I will give them a call next week though

Are you sure the transport lock for either the lid's transport mechanism or the base's transport mechanism are not partially engaging? Flip them back and forth to make sure.
