Voigtlander R2A Adjustment - What did I overlook?

So, I finally did my own DIY focus adjustment on my R2A after studying all the available instructions on how to adjust Horizontal and Vertical patch.

In the beginning, the vertical adjustment was a hair off (it was a bit higher). But after my adjustment, the horizontal and vertical patch are now perfectly aligned when the focus distance was set to infinity on both 35mm 1.4 and 50mm 1.4 (both Voigtlander lenses). The object that is in "infinity" is located many miles away from where I was standing; I chose a radio tower at the horizon...a good 7 miles away from my spot.

The camera was mounted on a tripod throughout the adjustment process.

However, when I took photos of object at 4 feet or 7 feet away, most of them came out FRONT-FOCUSED.
The shots were done at aperture 1.4

I promptly checked the H and V allgnment at infinity, and they are still allgned perfectly.

What did I miss?
What did I do wrong?

Btw, those two lenses can focus just fine on Leica M6 (at 1.4), no front or back focusing.
