For me the answer is easy, my little Nikkor 35/2 D lens. The few times I really need it there always seems to be an issue. I just sent it off for the 3rd time to have oil removed from the blades. This time I'm skipping Nikon Factory Service and sending it to their former facility in Chicago, APS, who did a great job on my 70-200/2.8 G lens.

Do you have a piece of gear that lets you down more often than not?

Today the Copal shutter in my Nikkor-W 240/5.6. Wouldn't run at 1/8, 1/4, 1/2 or 1 second without sticking, though I suppose I'm lucky that I've not had issues with a LF shutter before today. I'm letting it warm up to see if it was the cold today as it was running fine last week.

Last week it was my Pentax spot meter, a battery check Wednesday showed batteries fine, I returned to it Friday morning and batteries were dead, dead, dead. Welcome to working in Vermont winter temperatures...

On the opposite end of the spectrum I took out the very first lens I ever owned, a 50/2 HC Nikkor after a multi-year respite and it worked like a charm yesterday. I forgot how good that lens really is.
