do people use a case or bag when carrying one camera with a lens attached?
i prefer to just throw the camera over my shoulder but that isn't always a good thing.

For my M6 and lens I use a soft leather zipper bag with a wrist strap that was made long ago by Benser. There is just enough room for the M6 with 50 Summilux and one roll of film squeezed in alongside. The wrist strap allows me to almost hide the camera in my hand when I am carrying it. They are hard to find but turn up on e bay sometimes. Works well for me. Joe

I only use a case to protect the camera when I pack it in a larger bag. I'd rather use a small bag for the camera as well as small non-photo stuff.

We will have street photography exhibition in Toronto soon, where I could see Winogrand prints for first time. I would wear my M4-2. Same as I did in Montreal at George Zimbel exhibition. Going to places like this without camera is like going on military parade in pajamas. 
I do take one lens, one camera with me everyday.
If it is short trip to grocery store I wear it to be able to take shot almost immediately for short period of outdoor time. I would walk into the store with camera on me, but no pictures inside.
If I'm going for hiking or at daily work, camera is in the bag for transportation. It is not easy to wear camera and handle kids, items and dog for getting in/from the car. I also never wear camera at work. I'm using it if I walk to customer site and after work on site. It is not so awkward to walk with camera in the store, but strange to wear it at TV station.
I never wear it over my shoulder. It is most dangerous and too slow to get it quick. I do wear it over my neck or it is in the bag.
And case is same as condom. Adds protection, but no direct contact. In general
. FED-2 in original case makes me feel like pulling horse. And smells same as harness.
It was nice to have XA and Minox 35 in small soft case attached to the belt. Kind a Colt shooting. But those cameras didn't lasts for everyday use. I like small MF folder in the soft case, but it is slightly too slow for everyday use.

If it isn't either in my hand or around my neck, it's in a sling type bag of some type. Doesn't matter if it is bigger than what is needed for one lens and camera.. Having it in a non-descript bag is a good thing these days. One that doesn't scream camera bag that is, although I think one of my favorites, Think Tank Turnstyle, come pretty darn close t screaming camera bag as opposed to one of my converted messenger bags.

Usually a belt-bag, but then I have a sensible small camera!
When 'on the prowl' I use a wrist strap and camera to hand.
Just works for me - light weight and no strain on the back or neck!

Very rarely bring more than one camera with lens attached. Never use a strap so it's either in my hand or in my bag. Usually the bag is a messenger bag (Jack Sp ade the profanity check doesn't let you write that out) with a Crumpler insert.
For travel I bring this cheap Holga branded bag that holds a camera with lens attached and you can stick another lens in there if you want (it's got a little velcro secured divider). I've looked (and asked here) trying to find a better solution but it just kinda works.

I usually just have the camera with one lens around my neck - no bag. If it's a film camera, I have a meter and extra film in my jacket pockets. If it's too hot for a jacket, I carry a Domke 5xb (if just one camera). Bonus, that lets me bring another couple lenses without significant penalty.

I've been using camera bag inserts in my backpack lately. Recently purchased a Filson Small Pack. A Crumpler Haven pouch inside and it's perfect for what you mentioned.

For one camera that I don't want to carry naked, I use a simple world famous canvas hippy bag. Can hold an extra lens in a pouch, and plastic film cans.

I usually have a small cheap canvas shoulder messenger-style bag I got at the markets, with insert. Usually carry camera with wrist strap, only put it in bag when I've finished using it.

As I tend to use the one camera/one lens more and more often, I have started using ever-ready cases. The camera then goes into my normal shoulder bag with a lot of other stuff. In my 40 years of photography I have never done that before, but I have found it to be very practical indeed.
