I have an Angelo Pelle Henri in WWI brown. http://www.angelo-pelle.com/henri-bag.html
I have one in the mail too. First time I buy an expensive bag (more than $100), but I looked for the longest time for something that fit in my backpack when on business travel (m and a couple of lenses), and this one seems perfect all the way around.
I was on the verge of buying the original respective Leica case until I saw the modern Pelle version.

do people use a case or bag when carrying one camera with a lens attached?
i prefer to just throw the camera over my shoulder but that isn't always a good thing.
They say throwing salt over your left shoulder is good luck. A camera, not so much
I like those neoprene everready cases...protective, water-resistent*, and comes completely off and stuff in a pocket when shooting.
*I once had a camera in a neoprene case slip off my shoulder and land in a toilet bowl. No damage to the camera which stayed dry. The case was easily washed in the sink and dried quickly.
