The Perkeo I is probably one of the smallest 6x6.
For 645, various Zeiss bodies are very small.

Zeiss Nettar 515, or similar no-frills 6x4.5 bodies. Smallest in relation to film size, on a fully featured camera, is the Kamera Werkstätten Patent Etui 6.5x9 (and its many imitations) - sheet film, but then, only slightly bigger than a film sheet, and thinner than a 120 reel.

A copy of the small Zeiss... Zenobia
A copy of the small Zeiss... Zenobia
Virtually identical to the Zeiss 6X4.5 mentioned before. Made in Japan late 40's and early 50's by Daiiwa (Daiichi Optical Company). Two lens variations Hesper Anastigmat and Neo Hesper (later). Shutters D.O.C (Daichi Optical Company), DOC Rapid, and later Seikosha Rapid shutter. The DOC shutters suffer some reliability, but all the shutters shot to 1/500th. At the very end of production, there was a model with an uncoupled rangefinder... the Zenobia R.
Smallest folder I have seen. Shoots 6X4.5 in portrait configuration.
Three on eBay right now.. usually sell from $25 to $45.

Between the smallest, there isn't a heck of a lot of size difference. Basically, you needed a bellows, a lens and shutter, a film transport mechanism and a shell to enclose it and that is what all of the most stripped-down basic folders pretty much are. I assume we are discounting the 127 folders, so you're pretty much just looking for the smallest format you can get on 120, 6x4.5, because those could be made a little shorter than the 6x9, 6x7 and 6x6 formats. Just pick any bare bones zone focusing model in that format and they will all be pretty much the same size.

Comparing the best folders, the Perkeos are the smallest in 6x6.

Zenobia 6x4.5 is the smallest I've ever held.
