I ran a test roll through my new Canon FT QL camera and everything came out fine, except for one frame that sort of overlaps another by about a third. I seem to remember that at some point while shooting that I didn't quite fully advance the film on one shot, and when I pressed the shutter button nothing happened. After giving the advance crank a little bit of extra advance, I was able to fire the shutter. Unfortunately, I don't remember what shot that happened on. Might this have been the problem? Another FT QL I used to own did this now and then too.

I don't recall having that issue with mine. Better than it tearing the sprocket holes out. Just don't try to rapid-wind it so that your thumb slips off mid-stroke, and you should be okay.

The QL mechanism on these old Canons can sometimes mess up. Usually, it’s because the rubber “grab” pads on the take-up spool have lost their grip allowing the film to bunch up in the film chamber. I don’t know if this is the problem with yours, but I have seen this happen on a old Canon Pellix QL I own. The solution is to dab a bit of rubber contact cement on the pads. This will give them more grip and allow the film to wind tighter on the take-up spool.

Was it the last frame ? Film is numbered, unless you shoot bulk. Clutch failure on FT's is rare, a dirty rewind shaft bearing is common. Sprocket rotation is what advances the film the same amount per stroke, mis framing / overlap can be a sprocket shaft failure, but usually more than one frame. The film will tell you what happened, post up a contact sheet ?
